This week’s tip continues discussing the Analysis phase of FRACAS. As mentioned in last week’s tip, there are a number of Problem Resolution/Root Cause Failure Analysis (RCFA) processes and tools that are available.
Figure 1 below illustrates a typical process that has proven to be effective:
